Primary Health Care Provider Assistant
- Assists health care provider in the delivery of safe, efficient and quality primary care services
Primary Care Outreach Clinic
- Increase access to and utilization of primary care, mental health, and addiction services
- Provide counselling for substance abuse, through screening and identification, pharmacotherapy and treatment of complications
- Collaborate with the Manitoba HIV Program in delivering HIV care and prevention strategies
- Provide referrals to and network with, support services/resources/partners
- Foster local capacity building through knowledge sharing and inter-agency collaboration
- Also available to Sapotaweyak three to four times monthly. Please call SVPCC for more information.
Primary Care Pharmacist
- Supports medication review and management with chronic and complex health conditions
- Works together with patients and health care providers to ensure patients are on the best medication plan for management of their health
Community Mental Health Worker - Shared Care
- Provides brief treatment of up to 8 sessions for patients experiencing anxiety, depression, or adjustment disorders
- Referrals can be made by ones health care provider
- Sessions are focused on improving ones mental health status through a brief period of counseling and then connecting with the proper resouces if continued treatment is required or recommended
Nurse Practitioner
- Two Nurse Practitioners provide service at SVPCC
